Jerry G. Hernandez 

I am a practicing Family Nurse Practitioner who proudly received my training as a Nurse Practitioner from the University of South Alabama in May 2016 and became dual certified as a Family Nurse Practitioner and Adult Gerontologic Acute Care Nurse Practitioner.

 

In the practice of Gerontology, I have helped decrease risk factors while improving the quality of life for my aging patients.  Reducing fall risks (eliminating throw rugs, asking patients to stand briefly before starting to walk), as well as educating patients about the aging process has benefitted many along the path of maturity. Helping aging patients realize how metabolization of certain medications is different in the 7th decade on has helped avoid unwanted adverse effects of medications not seen in earlier stages of life. 

 

The Acute Care component of my work has been rewarding when I can help a patient with life threatening conditions stabilize so they can begin the healing and recovery process. 

Since graduation I have experienced positive outcomes with integrative approaches to holistic and allopathic medicine. I have been in the medical field since 2005 serving Charlotte County and Punta Gorda. I have been a resident of Charlotte County for the last 20 years. My passion is to learn enough about you as an individual patient to offer you full service, individualized patient care that you can understand and participate in.

 

I am passionate about soccer, health, and wellness. When not working, I enjoy spending time with my wife and three children. I look forward to meeting you and providing care that makes a difference!
